
Chapter FormsLocal Chapters are vital to PERI's ability to powerfully communicate our concerns to members of the Ohio General Assembly. A key element in this process is the timely communication of accurate information. Below are the forms that the Association needs in order to ensure the quick delivery of critical information. Chapter Officers Form - This form is the official notification to the Association's Main Office of your Chapter's leadership and organization. It details officer names, addresses, meeting months, days, times, locations; and other details of your Chapter's operation. It is to be filed out and mailed to the office no later than January 1 of each year, and any other time a change occurs. Chapter Meeting Report - This form is to be sent to the main office as a cover to meeting minutes. The information provided relates details regarding each Chapter's activities and growth. The District Representative receives a monthly summation of these reports in order to help all Chapters learn from the success of others. |
