Chapter FormsLocal Chapters are vital to PERI's ability to powerfully communicate our concerns to members of the Ohio General Assembly. A key element in this process is the timely communication of accurate information. Below are the forms that the Association needs in order to ensure the quick delivery of critical information. Chapter Information Report - This form is the official notification to the Association's Main Office of your Chapter's leadership and organization. It details officer names, addresses, meeting months, days, times, locations; and other details of your Chapter's operation. It is to be filed out and mailed to the office no later than January 1 of each year, and within 30 days of any change in banking, officer, or meeting information.. Chapter Meeting Report - This form is to be sent to the main office as a cover to meeting minutes. The information provided relates details regarding each Chapter's activities and growth. The District Representative receives a monthly summation of these reports in order to help all Chapters learn from the success of others. Chapter Financial Report - This standard form is used by many Chapter Treasurers to report their financial transactions between meetings, and for their year end summary. Click here for a copy of the Chapter Financial report Chapter Audit Directions and Report - Each year chapters select an audit committee that is responsible for checking the bank accounts and financial records and proceedures of the chapter. Chapter Bylaws - This document contains the purpose and operating rules for all of our local chapters. Click here to read the Chapter Bylaws Chapter Legislative Action Network Forms - Each chapter member may sign up to contact one or more of their local legislators when it becomes necessary to do so. These forms are then given to the chapter's legislative officer, who in turn, assigns the member to one of the chapter officers. The chapter officers will contact the members on their legislative roster, when legislative action is requrired. Click here for a copy of the Member sign up form.
